[ARCHIVED] Back-to-School Immunizations

The Williamson County Health Department encourages parents to have their school-age children immunized now to avoid long lines and extended wait times that may occur closer to the start of the 2015-2016 school year.

Here are the requirements for school vaccinations in Tennessee:
• Kindergarteners and other children enrolling in a Tennessee school for the first time must provide schools with a complete Official Tennessee Immunization Certificate before classes begin. The certificate must be signed by a qualified healthcare provider or verified by the state’s immunization information system.

• All current students entering seventh grade are required to give the school a limited Official Tennessee Immunization Certificate showing they have had a second dose of chickenpox vaccine (or a history of the illness) and a booster shot for tetanus, diphtheria.

Please print and complete the Patient Demographic form (add hyperlink) and bring a copy of your child’s immunization records to the health department. Our hours are Monday – Friday, 8:00 a.m. – 4:30 p.m.
